The "Dog-Tag" System consists of a highly reflective soft, tough neon yellow (recommended safety color for visibility and safety) tag that contains a rear compartment that carries the medical data/ID form.
This "Dog-Tag" can be worn around the neck with either a metal chain or breakaway safety lanyard. There is also an additional buttonhole for versatility in attachments or emergency situations. This entire System is waterproof. We are including a pencil in the kit to fill out the information.

How To: Instructions for Using This System

1. Using a pencil, print all pertinent information on the waterproof, durable Medical data/ID form.

2. Fold Data Form in half, then in thirds. Make sure your name and the MICS logo appear on an outside panel. |

3. Slide the Data Form into the orange MICS carrier so that your name and the MICS logo are visible through the clear part of the carrier. Fold the tab inside the carrier to secure the Data Form. |
